小志愿者流程

Steps to appoint a Minor as a 志愿者

1. Inform minor volunteer that the following will be required attachments to the volunteer 形式:

   2. 完成 State of California Department of 教育 Permit to Employ and Work form [pdf]. The form must be signed by the school the volunteer is currently attending.
      1. If the volunteer's school requires the Statement of Intent to Employ a Minor and Work Permit Request form [pdf] to be signed, it must be signed by Appropriate Administrator (MPP) that signed off 在志愿者申请表上.
      2. If the volunteer is a minor with a high school diploma, the parental consent form is still required but the work permit is not required.
2. 完成 志愿者 Request Form [DocuSign].
3. Once the 志愿者 Request Form is received and reviewed, 大学工作人员 will reach out to the supervisor regarding a 生活 Scan Fingerprinting. 这是一个要求 per the University's 志愿者 Policy.
   1. 菠菜网lol正规平台 does not run 生活 Scans on Minors.
4. After review and approval from 大学工作人员, the volunteer appointment will 进入PeopleSoft.
5. Once the completed 志愿者 Request Form is received, check the 大学工作人员: 志愿者 Appointments section on the form to verify the status of the volunteer request.
